Education Overview

In this second session of a three-part series on the new QAPI initiative, Diane Link, RN, MHA of BlackTree Healthcare Consultants will provide a detailed analysis of how using the data contained in your CASPER Report can help you reduce your Acute Care Hospitalizations. The learning objectives for this webinar are:

  • Identify data sources available to identify causes of acute care hospitalization and emergency room utilizations
  • Identify OASIS identifiers
  • Identify process improvement areas for risk factors for hospitalization
  • Root Cause Analysis
  • Performance Improvement Plans

There will be an Implementation Checklist and CASPER guide that you will receive as part of your Registration for this webinar.

About the Educator

Diane Link, RN, MHA, BlackTree Healthcare Consulting - Diane Link has been a RN for over 25 years. She currently lives in Gettysburg PA and is a Senior Clinical Consultant for home health and hospice with BlackTree Healthcare Consulting. Diane has a Master’s Degree in Healthcare Administration from Mount Saint Mary’s University in Maryland and maintains her RN license in Maryland, Pennsylvania and New Jersey. She has a Green Belt in Lean Six Sigma. 

Diane is president of the board for the Maryland National Capital Home Care Association. She has received awards from the MNCHA over the years for her leadership and health care policy advocacy. Diane has published the Emergency Preparedness Manual for Home Care Providers in Maryland.
