Over 28.2 million adults are diagnosed with heart disease that makes up 11.5% of the US adult population and these are only the ones that are diagnosed. As care providers we need to understand cardiovascular disease, early identification and treatment options so that we can educate our patients.
All home health patients are at risk for skin integrity issues especially our senior patients. In this session we will review the changes that occur in skin over the ages, the most common skin disorders and how the nursing team can work together to prevent skin breakdown.
Patients and Payer sources are using the CMS Five Star Ratings and Home Health/Hospice Patient Satisfaction Scores to provide a glimpse into the agency’s quality of patient care. In this presentation we will look at the role of the Home Health Aide in Outcome Improvement and Patient Satisfaction including tips for the home health/hospice aides on how to implement changes in their care practices to help move outcome scores.
CMS Targeted Probe and Educate was implemented in October 2017. The Medicare Administrator Contractors (MACS) have released their initial findings from the first round of TPE with some familiar themes. Agencies can use this information to prepare for future ADRs.
